Referring to a Dietitian is now quicker and easier than ever before!

Using the new electronic referral form developed by the Dietitians Association of Australia (DAA), referring to an Accredited Practising Dietitian (APD) is very quick and very easy! Access the form via the DAA website and save it straight into Practice software. There is a version compatible with Medical Director software and one compatible with Best Practice software.  An example template is also available with automatically populated fields indicated by <<ABC>>. 

Key benefits of the form include: 

  • Can be used to refer patients eligible for Medicare rebates
  • Developed with APDs who currently work with GPs
  • Automatically populates patient, GP & dietitian details
  • Automatically populates information relevant to dietetic consultation

The form is concise and at the same time provides adequate details to help APDs deliver appropriate nutrition advice, which will assist in providing best possible patient care.
