Making social workers part of the GP team

GPs considering referring a patient to an Accredited Mental Health Social Worker (AMHSW) as part of GP Mental Health Treatment Plan or Chronic Disease Management Plan and Team Care Arrangement may be interested in the resources recently developed by the Australian Association of Social Workers. These resources have been developed to enhance GPs understanding of Accredited Mental Health Social Workers (AMHSWs) training and skills, where to find one, and the services they can provide to assist GPs to improve patient outcomes.
