Dementia Support Pharmacists

The Pharmaceutical Society of Australia (PSA) South Australia and Northern Territory Branch is seeking support from AMA SA members for its Dementia Support Pharmacist program.

The PSA says the Dementia Support Pharmacist program, which is funded though the Country SA Public Health Network (PHN), is dedicated to transforming dementia care across community, home settings, and aged care facilities.

Comprehensive Medication Management: Dementia support pharmacists provide expert guidance on medication usage, adverse effects management, polypharmacy, and compliance to ensure optimal treatment outcomes. 

Personalised support: Through both face-to-face and telehealth consultations, dementia support pharmacists empower clients and families with tailored education, symptom management strategies, advocacy for the person living with dementia, and advanced care planning, promoting independence and quality of life. 

Collaborative care: Pharmacists involved in the program forge strong partnerships with specialised nurses, general practices, and aged care facilities, establishing local networks that enhance communication and streamline healthcare delivery. 

Enhanced quality of life: By reducing hospitalisations and supporting end-of-life care, the program enables clients to live well with dementia in their preferred environments for longer periods. 

Free service across regional South Australia: The program offers a no-cost service to anyone diagnosed with dementia, regardless of age, throughout regional South Australia. It provides comprehensive symptom support, medication management, and education, with a focus on palliative care when needed. This service extends not only to clients living with dementia but also supports families and associated healthcare professionals in their caregiving journey. 

The PSA says it looks forward to working closely with GP specialists and non-GP specialists throughout Country SA PHN to provide those living with dementia multidisciplinary healthcare that helps them to stay in their desired location for longer and enhances their quality of life.
